JOB SUMMARY:

The Registered Nurse (RN) will provide professional nursing care to patients following established standards and practices as a member of the Neighborhood Health team. 

Promote, protect and preserve the health of the public through:

  • promoting healthy lifestyle
  • prevent disease and health problems
  • provide direct care
  • educate community about managing chronic conditions and making healthy choices

Assess and evaluate community health needs including future needs; plan appropriate interventions for identified health needs of the community; provide comprehensive nursing services on health promotion/illness prevention and counseling. These activities contribute to meeting the health care, safety and comfort needs of patients and are performed in accordance with all existing regulatory, state and company standards, policies and procedures.

JOB RESPONSIBILITIES

  1. Responsible for collaborating with the staff for the day-to-day operations of the clinics:
  • Site set up and break down
  1. Provides safe and appropriate patient basic care and administrative duties in order to assist patients in achieving established outcomes by: collecting data points;
  • Vital signs: Weight, Temperature, HR, BP, RR, Pulse Oximetry, Pain
  • Allergies
  • Confirm all medications and reconcile medications; document pharmacy location if available
  • Past Medical History
  • Point of Care Testing (POCT)
  • Administer IV and PO medications and SQ vaccines
  • Perform venipuncture for Complete Metabolic Panel assessment
  1. Ability to delegate responsibilities to appropriate team members based upon an understanding of other’s roles, knowledge and scope of practice
  2. Perform various other duties as assigned
  3. Follow standard work
  4. Maintains a safe, clean, and orderly environment for patients and colleagues
  5. Contributes to improvement by participating in huddles, training, education and performance improvement activities as directed
  6. Models quality patient/customer services by:
  • Demonstrating a willingness to serve all customers at all times
  • Maintains an awareness of patient needs and utilize the initiative to respond to those needs within skill level
  • Utilizes tact, discretion and compassion in collaborating with other members of the team to ensure quality patient care
  1. Initiates appropriate action in urgent situations by adjusting priorities to address customers and staffs immediate issues.

Qualifications

Qualifications

Required Education:

  • Current RN license in the state of Connecticut
  • Bachelor’s Degree in Nursing
  • MSN Preferred

Required Experience:

  • Minimum of 3 years as a registered nurse
  • Professional nursing experience in an ED hospital, medical office, or ICU setting
  • Triage experience
  • Venipuncture experience
  • Narcan administration experience
  • Behavioral health experience preferred
  • Licensure, Certification, Registration:
  • BLS certification required
  • Bilingual preferred

           - Excellent communication skills demonstrated through written documentation, direct communication and using active listening skills

- Customer service skills: proactively and promptly responds to customer needs
