The Institute of Living, one of the first mental health centers in the U.S., remains one of America’s leading not-for-profit centers for patient care, research and education in the fields of behavioral, psychiatric and addiction disorder. Through our dedicated, experienced and skilled clinicians, we provide patients and their families with the highest quality of personalized care. Programs are offered for children, adolescents and adults struggling with mental illness as well as offering services specializing in Anxiety, Eating Disorders, and Geriatric Services.
Job Summary
Provides holistic, quality and cost-effective advanced practice care for patients, families, and others in a target population, and participates in the medical management of patients, to achieve or maintain optimal health status, manage disease and reduce risk of illness. Engages in independent decision-making regarding the health care of individuals, families and groups across the lifespan and across the health care continuum. Reports to Nursing Director, Medical Director or Nurse Manager, or a matrix which may include Nurse Manager, Nursing Director, Medical Director, or some combination of those.
Job Responsibilities
Assesses health status, illness conditions, responses to illness, coping behaviors, environment and health risks of individuals, families, and groups in order to diagnose actual or potential health status or disease state. These assessments are accomplished by taking histories, performing physical examinations, ordering and interpreting laboratory tests/data.
Plans therapeutic interventions jointly with the patient and family to achieve established outcomes. Types of intervention may include health promotion, disease prevention, treatment of illness, chronic illness management, or limiting disability. These are accomplished by prompt treatment, direct care, prescription of medication or other therapies, consultation, and referral, patient, family and population education and case management.
Engages in /collaborates with others in the research process to discover, examine and test knowledge, theories, and creative approaches to practice. Uses research findings as a basis for practice and to improve health care.
Provides consultation to health care providers and others to influence care plans, enhance ability to provide health care, and affect change in systems.
Educates staff and students, formally or informally, to enhance their ability to provide patient care.
Demonstrates competencies, including knowledge and behaviors, to meet age-specific patient needs (e.g. Pediatrics, Adolescents, Geriatrics.)
Models quality patient/customer service by: demonstrating a willingness to serve all customers at all times, maintaining an awareness of patient/customer needs and responding to those needs; maintaining confidentiality of all patient information and functioning in a collaborative manner with other care-givers to ensure quality patient care.
Contributes to care improvement by participating in : coaching, mentoring and developing staff, performance/quality improvement opportunities, and research the cost-effective utilization of resources. Translates relevant research findings into practice.
Assumes responsibility for self improvement by identifying, communicating and seeking resources or instruction to meet self-learning needs.
Qualifications
Completion of Approved Nurse Practitioner Program if appropriate for specialty.
Current APRN license required after 1994.
Certification according to CT state law.
Masters Degree in Nursing strongly preferred
Specific experience in specialty may be required.
