The creation of the HHC System Support Office recognizes the work of a large and growing group of employees whose responsibilities are continually evolving so that we and our departments now work on behalf of the system as a whole, rather than a single member organization.

JOB SUMMARY:
The Registration Associate will interview patients to obtain/verify personal/demographic information, emergency contacts, primary care physicians, insurance information, the person who is financially responsible for treatment, and the required health equity mandatory data set.
These activities contribute to meeting the health care, safety and comfort needs of patients and are performed in accordance with all existing regulatory, state and company standards, policies and procedures.
JOB RESPONSIBILITIES:
1.Registers/admits patients to the clinics by obtaining/verifying personal/demographic information, emergency contacts, primary care physicians, insurance information, financial information, and the required health equity mandatory data set and obtains signatures on consent-to-treat
2.Facilitate lines of communication between Neighborhood Health team to ensure seamless registration and admission process.
3.Assists with information gathering of required Social Determinants of Health (SDOH) and General Anxiety Disorder (GAD).
4.Models quality patient/customer services by:
-Demonstrating a willingness to serve all customers at all times
-Maintains an awareness of patient needs and utilize the initiative to respond to those needs within skill level
Utilizes tact, discretion and compassion in collaborating with other members of the team to ensure quality patient care
5.Assists in orientation of new staff members as requested; assists and collaborates with colleagues
6.Initiates appropriate action in urgent situations by adjusting priorities to address customers and staffs immediate issues.
7.Responds to changes in work schedules with flexibility as necessary or as requested, including weekends, evenings as well as varied site locations
8.Demonstrates competence in all procedures required
9.Inspires confidence from patients and visitors by performing and communicating in a professional manner at all times with ability to connect with various types of patients (such as children, elderly) and make them feel comfortable
10.Cross trained in Screening Role
11.Perform various other duties as assigned
