Hartford Hospital Hospitalist Service
Our hospital provides an incredible impact to the healthcare of Connecticut’s residents. In the Hospitalist service we work to lessen waiting in the emergency room, speed up diagnosis and treatments, reduce hospital stays while improving outcomes and continuity of care. Hospitalists provide support for Primary Care Physicians and rapid response to all hospitalized patients.
Hospitalist - Manager of Advanced Practitioners
Directly manages and develops staff and oversees daily operations of clinical staff within Hospital Medicine that provides education, health promotion and support to patients. Responsible for oversight of hiring, scheduling, assignments, coaching, mentoring, training and education. Provides quality, holistic and cost-effective advanced practice care for patients, families, and others in a target population, and participates in the medical management of patients, to achieve or maintain optimal health status, manage disease and reduce risk of illness. Is actively involved in department quality and safety and is a critical member of the Hospital Medicine leadership team.
Plans appropriate therapeutic interventions jointly with the patient and supervising physician and collaborative team to achieve established outcomes. Types of intervention may include health promotion, disease prevention, and treatment of illness, chronic illness management, or limiting disability. These are accomplished by prompt treatment, direct care, prescription of medication or other therapies, consultation and referral, patient, family and population education, and case management.
Collaborates with the management team to support and develop initiatives and business plans with fiscal and regulatory responsibility.
Models the effective, efficient operation of the department by scheduling and assigning work, resolving process problems, collecting data to monitor progress, and carrying an assigned workload or filling in as necessary. Continually strives to improve quality and productivity by identifying improvement opportunities and recommending and implementing changes.
Assists the collaborative management team in ensuring staff compliance with mandatory staff training and documentation.
Determines ongoing staff education needs and assists in the planning/coordination of educational programs to meet these needs
Implements and monitors departmental goals and objective for the work group, that aligns with the strategic direction of the organization, by analyzing needs and developing recommendations. Implements individual performance expectations; coaches, develops, and provides input to performance evaluations to enable employees to be successful in contributing to the achievement of individual and work group objectives.
Uses resources effectively and efficiently to achieve the goals and objectives of the work group within established budgets by recommending budget expenditures, monitoring progress, and shifting resources to meet the needs of the department.
Where appropriate and necessary, serves on department/hospital/system-wide committees to help promote patient safety, enhanced patient care, and other balanced scorecard initiatives.
Maintains effective, positive customer service relationships by responding to customer needs, resolving conflicts, and educating employees on customer service. Effectively and continually communicates with the staff, management and customers to facilitate the flow of information necessary to the success of the employees and the work group.
Actively seeks opportunities to model teamwork through collaboration both within and outside the work group in support of the organization’s objectives.
Qualifications
Advanced Practitioner Requirements
Graduate of an accredited Physician Assistant program or Completion of Approved Nurse Practitioner Program if appropriate for specialty.
Minimum of 3 years advanced practice experience in acute care or in specialty area.
Current CT APRN license or Current PA license.
Additional certification/training typically required related to complex chronic disease, limiting disability, and/or complex clinical condition per program requirements
Good verbal, written and interpersonal skills.
Current demonstrated clinical competence in advanced practice role
