Job Summary:

The Connections Coordinator supports the Stronger Families, Stronger Future (SFSF) grant-funded program administered by the Connecticut Office of Early Childhood. The Connections Coordinator screens families in the prenatal and/or postnatal period using a standardized tool to help decide which of the three program components best meets the needs of each family.  The three components are the following:  Telephone support, Prenatal groups and Home visiting program.

Job Responsibilities:

  1. Utilize eligibility criteria and standards to Identify, engage and refer eligible pregnant and postnatal mothers in early childhood home visitation services within prenatal clinics, hospital-based, NICU units and the community.

 

 

  1. Provide follow-up calls to prenatal or postpartum families for a minimum of three to six months following the birth of their child. The purpose of the call is to give the parents emotional support, information, and assist and or facilitate access to additional services as needed.
  1. Enter and maintain screening and referral data using Excel, Office of Early Childhood ECIS database, APRICOT 360 and EPIC.  As well as completing weekly and quarterly reports to the Village for Families and Children’s.
  1. Communicate and collaborate with hospital social workers to meet perinatal families in need of services and resource available in the community
  1. Provide emotional support and guidance on topics such as perinatal health, newborn states of consciousness, infant crying, safe sleep, bonding and attachment, infant and child development in a variety of settings that may include:           
  1. Prenatal Clinics
  2. Hospital
  3. Groups

 

 

  1. Identify and build connections with parenting resources within the site service delivery area. Make referrals to other statewide programs as needed.

 

  1. Recruit families to participate in prenatal groups. Groups are facilitated twice a week, English and Spanish for 5 weeks in the community.  Maintain attendance of each group and completing evaluations at end of each group.
   

 

Qualifications

  • High School Diploma. Associates Degree in Health Services relate field preferred.
  • At   least two years of experience in Human Services field and/or working with parents of infants and toddlers.
  • Use of hospital EPIC systems, Parents as Teachers curriculum, providing breastfeeding education and support. Familiar with Community Based Services in designated region preferred.
  • Bilingual- Fluent in English and Spanish preferred.
