The Community Health Navigator is responsible for providing outreach education, health assessments and community navigation services to identify, screen and provide risk reduction strategies to qualified program participants through Charlotte Hungerford Hospital’s Early Detection Program (EDP). The position is grant funded through the Department of Public Health’s CT Early Detection & Prevention Program (CEDPP). Responsibilities include but, are not limited to:
Promote enrollment in Affordable Care Act (ACA) market-based insurance plans
Provide health education & outreach activities to individuals in eligibility priority populations
Collaborate with local community agencies to identify potential underserved participants
Enroll eligible participants into the CT Early Detection & Prevention Program (CEDPP).
Assess eligible participants for barriers preventing participation in early detection screening and prevention services
Complete a baseline assessment for each CEDPP participant that includes a physical (BP & finger stick cholesterol testing) and health behavior evaluation
Complete all required assessment and screening documentation
Deliver risk reduction counseling as prescribed by the CEDPP policies and procedures
Support participants to identify and implement strategies and goals to reduce health risk behaviors
Work closely with medical providers and clinical personnel to ensure that clients have integrated, comprehensive, coordinated healthcare
Accompany Spanish speaking participants to medical office visits and screening services when required
Assist program participants with arranging appropriate transportation to appointments
Attend all required program meetings, DPH provider meetings and training sessions

Qualifications
EDUCATION:
Minimum High School Diploma or equivalent.
Medical Interpreter Certification or willingness to obtain certification within one year of hire.
EXPERIENCE
2-3 years of experience in a medical setting preferred.
KNOWLEDGE, SKILL REQUIREMENTS, AND DESIRED ATTRIBUTES
Bilingual English/Spanish highly preferred.
Excellent verbal and written communication skills a must.
Basic clerical and personal computer skills are required.
Flexibility in work hours including some availability evenings and weekends when needed at community events.
Ability to manage multiple priorities and ensure all grant program requirements are followed.
