The creation of the HHC System Support Office recognizes the work of a large and growing group of employees whose responsibilities are continually evolving so that we and our departments now work on behalf of the system as a whole, rather than a single member organization. With the creation of our new umbrella organization we now have our own identity with a unique payroll, benefits, performance management system, service recognition programs and other common practices across the system
Establishes and maintains collaborative processes with internal constituents and community partners around community benefit capture, reporting and articulation. Performs their duties in a way that supports the vision and mission of HHC, and in particular the work of providing a fair and just opportunity for all of those we serve to lead healthy and productive lives, and ensuring the communities we serve are reaching their potential socially, economically, and with respect to health outcomes.
Key areas of responsibility
Oversees all hospital CBISA databases, handles all communications with Lyon Software and disseminates any information on changes or upgrades to CBISA Coordinators, provide training to Coordinators in order to keep each hospital’s database current and support reporter onboarding/training as needed. Support development and updates on education and awareness campaigns for departments and employees.
Liaison with Finance and Tax Department to ensure fringe percent, in-direct costs and salary averages are entered into CBISA on a timely basis and support regional communications for Schedule H narratives. Provides any reporting needed from the CBISA database including assisting with required OHS CB Annual Reporting. Liaison with CHA for CBISA reporting.
Provides support to regional colleagues conducting a community health needs assessment and ongoing support during development and continued execution of HHC’s regional community health improvement plans. Supports regions in compliance with HHC’s CB Policy.
Attends system Community Health & Engagement Team (CHET) meetings on a bi-weekly basis to share and promote best practices with those in community outreach positions across HHC. Supports coordination of meeting content and delivery and provides leadership in driving change and executing system-wide initiatives that flow from HHC’s CHET.
Be conversant in administration role for HHC’s Connections That Matter (CTM) platform. Assist in training/troubleshooting for HHC colleagues’ and community partners’ with the utilization of CTM.
Performs other related duties as required
REQUIREMENTS AND SPECIFICATIONS
Minimum 5 years’ experience in a related field, health or social sector
Personal lived experience with intersectional and marginalized identities a plus.
It is strongly preferred that candidates have experience with conducting Community Health Needs Assessments and developing and executing Community Health Improvement Plans
Bi-lingual a plus

Demonstrated commitment to inclusive leadership

Exceptional writing and communications skill
Project management/coordination skills; knowledge of non-profits, demonstrated volunteerism a plus
Exceptional computer skills (PC preferred with Microsoft Office – Word, Excel, PowerPoint);
Exceptional interpersonal skills with the ability to translate complex data and concepts into user-friendly information that is relevant across disciplines for diverse stakeholders
