Position Summary:

The Senior Director of Care Continuum Integration, being an integral member of the HHC Care Management Executive Leadership Team, leads the development, delivery and optimization of ambulatory Care Management services while also leveraging technology and data to support integration across all care continuum access points.  The Senior Director will lead the change process required to implement the system’s model of Care Management and Population Health, including infrastructure changes, plans for new skills acquisition, expanded relationships with community resources, and achieving clinical documentation excellence.

Key Accountabilities:

  • Leads the care management strategic initiatives within the ambulatory environment in partnership with primary care and specialty care by supporting operational leaders within the ambulatory care management team to implement initiative related workflows and processes
  • Ensures that technology and software systems support workflow with clinical logic, standards of practice and regulatory and accreditation requirements
  • Builds, promotes, and educates to workflows and processes that support down-side risk and capitated populations
  • Serves as the domain expert for any activity that relates to clinical care management workflows
  • Provides guidance on clinical best practices related to use of available technological tools and enhancements in order to evaluate their value and efficacy within clinical care management workflows
  • Serves as the central resource for the executive team concerning strategies which are patient-centered, efficient, effective, and support appropriate financial reimbursement
  • Creates data priorities and establishes dashboards for monitoring outcomes
  • Develops structures and processes that contribute to continuity of care through demonstrated use of the interdisciplinary team approach to patient care across the care continuum
  • Works collaboratively with internal and external resources and agencies, physicians and all members of the interdisciplinary team in meeting customer needs and organizational goals


  • Master’s degree in a healthcare related field is required
  • Current Connecticut licensure or certification in field of study is required (i.e. RN, PA, LCSW, PT, OT, etc.)
  • Ten plus years of clinical experience is required
  • Ten plus years of Case Management experience is required
  • Ten plus years of progressive leadership experience is required
  • Executive presentation and excellent interpersonal communication skills, relates well with all levels of the organization
  • A creative, forward thinking leader with the proven ability to engage, coach and mentor direct reports, colleagues and other key stakeholders
  • Ability to interact and negotiate with multiple stakeholder groups including providers, payers, practice management staff
  • Highly effective analytical, organizational and problem-solving skills
  • Positive customer service focused approach
  • Must be comfortable with ambiguity and change and capable of supporting staff through transitions
  • Strong analytical skills and ability to translate analysis into process improvement opportunities
