Physician - Per Diem SNF Admission Coverage

Position Details:

Hartford HealthCare is seeking a per diem Internist, Family Medicine or Geriatrician on a per diem basis to provide Skilled Nursing Facility (SNF) admission coverage.   

Role: Geriatrician

Location:  2-3 local SNF’s (locations mostly in the greater Hartford area)

Setting: 100% Skilled Nursing Facility (SNF) – admitting patients to Short Term Rehab and Long Term Care and attend to complex acute issues

Schedule:  Per Diem, as needed (seeking 4-5 hours/day, 3-4 days a week to cover provider vacations

Call:  No call

• Supported by team of APP’s that are in the facilities full time within a team of 18 Geriatric providers
• Strong support staff, nursing and operational team plus strong peer support to assist with any challenging situations that may arise
