Job Summary

Provides quality, cost-effective inpatient/outpatient care for patients/families and/or others in a target population across the continuum of care by assuring access, providing assessment, coordinating required resources, and partnering with health care team members.  By utilizing excellent communication and negotiation skills, focuses on providing the appropriate service in the most cost-effective formula to assure that the patient, provider, payor and health organization’s goals are met.  Reports to Unit Manager/Program Manager.

Job Description

  1. Assesses and plans patient care by:  obtaining, assessing and interpreting the psychiatric and psychosocial needs of the patient; identifying and documenting problems and strengths utilizing scientific knowledge base and appropriate psychological/social/cultural concepts; organizing and prioritizing care and interventions; and identifying, obtaining and stewarding appropriate resources in order to meet individual and family goals.
  2. Provides direct patient care utilizing individual, group and/or family therapy modalities; and through advocacy, referral and discharge planning to meet patient/family needs.
  3. Coordinates and implements patient/family care along the continuum by collaborating with the multidisciplinary team, other payors and partnering with patients/families; establishing a multidisciplinary plan to address identified issues and goals; documenting and communicating in a timely manner; and educating patient/family in order to meet established goals and ensure seamless, coordinated transitions across levels of care.
  4. Regularly evaluates goal directed effectiveness and efficiency of plan, documents patient/family responses, identifies variances from expected outcomes, and revises plan of care to meet patient needs.
  5. May provide supervision or clinical supervision to Treatment Manager I or other clinical staff as assigned.
  6. Works effectively as team member and contributes to improved patient care by:  participating in performance/quality improvement opportunities, research, and by coordinating activities such as meetings to improve patient care.  Ensures the cost-effective utilization of resources.
  7. Demonstrates competencies, including knowledge and behaviors, to meet age specific patient needs.
  8. Assumes responsibility for self improvement by identifying, communicating and seeking resources or instruction/validation to meet own learning needs, and is self-directed in maintaining currency of validations.


  • LCSW, PhD, or PsyD licensed to practice in the state of CT
  • Master's Degree is required
  • Ability to meet requirements for internal credentialing in individual and group or family therapy required.  If assigned to school programs, must possess appropriate Department of Education certification, or be certification eligible
  • Demonstrates competencies, including knowledge and behaviors, to meet age specific patient needs.
