Hartford HealthCare St. Vincent’s Rehabilitation Network provides rehabilitation services across the continuum - acute care hospital, inpatient rehab unit, homecare, and outpatient centers. Our new St. Vincent’s Milford Outpatient Rehabilitation site is one of over 40 Hartford Healthcare outpatient locations throughout the state offering physical, occupational, and speech therapy, in specialties including orthopedics, neuroscience, oncology, women’s health, sports medicine, vestibular, and more. Overview:
Hartford Healthcare St. Vincent’s Rehabilitation is seeking a Site Supervisor for its new Milford location. The ideal candidate will have experience in the outpatient setting and demonstrate strong leadership skills and a passion for multi-faceted patient care, including outpatient neuro, orthopedics, headache, back/neck pain, gait/balance, and vestibular conditions. In addition to maintaining direct patient care responsibilities, the Site Supervisor ensures that high quality, cost effective services are delivered in support of the organization’s strategic plan, goals and objectives.
The Milford Site Supervisor is a licensed Physical or Occupational therapist and directs on-site staff and activities of the team to provide outcomes-driven, high-quality care consistent with best practice. The supervisor will be a strong communicator and motivator, and will cultivate team collaboration. This clinician leader oversees daily functions of rehab staff, patient service representatives, and operations of the site while demonstrating characteristics of independent thinking that reflect a commitment to exceptional customer service and the achievement of overall company strategic goals.
This position will require program development, physician relationship building and community marketing and outreach. This therapist will interact with rehabilitation professionals throughout
the HHC system of care to ensure that best practices are followed in the provision of care. Data will be analyzed and utilized to improve patient outcomes, and the clinician will partner with program managers to implement, enhance, and optimize clinical programs. A Lean operational model will be followed to improve communication, team engagement, operational efficiencies, and continuous improvement.

Position Specifics
The site supervisor will also provide direct patient care: independently evaluates, determines treatment strategies, delivers and/or delegates services that identify, prevent, remediate, and rehabilitate acute or prolonged physical dysfunction or pain, with emphasis on physical dysfunction. This role encompasses evaluation skills and techniques, analysis of patients, and therapeutic application of physical and chemical agents, exercise and other procedures to maximize the patient's functional independence. Evaluation involves strength, motor control, joint mobility,
soft tissue integrity, pain, balance, coordination, posture, cardiopulmonary function, endurance, gait and functional abilities in activities of daily living.
