Hartford HealthCare is seeking an experienced, full-time Nurse Practitioner or Physician Assistant to join an established, multi-provider, multi-specialty, primary care team in Stamford, CT seeing patients of all ages.

Advanced Practitioner (APP) Role Scope:
Under established guidelines and in collaboration and/or supervision with the practice physician(s), performs routine physical examinations, and formulates medical, educational, and psychosocial plans to treat patient. In consultation with the physician(s), develops plan for further evaluation of health needs, e.g., referral to other health team members or specialists. Provide appropriate treatment for problems within scope of a Nurse Practitioner (APRN) or Physician assistant (PA) practice as designated by protocols. Identifies problems outside of APRN or PA scope of practice and refers appropriately to physician.
