Hartford HealthCare, the largest and most comprehensive healthcare system in Connecticut, has a new opportunity available for a Nurse Practitioner or Physician Assistant, to join our pre-admission testing team practicing in an ambulatory setting at the Bone & Joint Institute, New England’s premier orthopedic hospital, which is located on the Hartford Hospital campus in Hartford, CT.

 

Some highlights of this opportunity include:

Provides holistic, quality and cost-effective advanced practice care for orthopedic clinic patients and elective surgery patients in need of modification of co-morbid conditions in order to contribute to improved quality outcomes. Contribute to research database and patient teaching. Engages in independent decision-making regarding the health care of individuals, families and groups and program development. Reports to APRN Manager.

Details:

Advance the BJI Modifiable Risk Factors Program (MODIFY) at BJI in order to contribute to improved post-operative outcomes. Work collaboratively with Surgeons, Clinical Leaders and Community practitioners in order to identify best practice programs in order to support patients and family members.

Provide care to patients in the BJI Orthopedic outpatient clinic. Assesses health status, illness conditions, responses to illness, coping behaviors, environment and health risks of individuals, families, and groups in order to diagnose actual or potential health status or disease state. These assessments are accomplished by taking histories, performing physical examinations, ordering and interpreting laboratory tests/data.

Performs standardized preop assessments

Qualifications

Eligibility to be licensed in Connecticut as a Physician Assistant (PA), or Advanced Practice Registered Nurse (APRN), is required. Eligibility to be certified as a Physician Assistant (PA), or National Board Certification as a Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), or Adult Gerontology Primary Care Nurse Practitioner (AGPCNP) is required. Experience practicing as an Advanced Practitioner is required. Orthopedics, surgery, or primary care experience is highly preferred.
