Department Description:
Integrated Care Partners (ICP) is a physician-led, clinically integrated health care network whose mission is to assist our members in providing the highest quality care at the most reasonable cost for our patients in the current, rapidly changing medical environment.
Position Summary:
The Practice Transformation Specialist develops implements and evaluates Population Health Management strategies and tactics across the ambulatory environment. The Practice Transformation Specialist works to drive improvement in health outcomes, quality of care, and experience of care as well as increased engagement of providers and thereby realize enhanced value of care for the population of patients served by Integrated Care Partners/HHC. The Specialist will operationalize and manage care transformation at the practice level by leveraging data monitoring and analysis, relationship building with practice staff as well as Providers, and change management strategies.
Position Responsibilities:
- Drives practice engagement by sharing performance data and assessing current practice processes, mutually identifying areas of opportunity
- Through analysis of available performance data, collaborates with practice staff to develop practice-redesign interventions and programs incorporating best-practice and evidence based research that will drive Population Health Management (PHM) and improved quality outcomes
- Implements practice-redesign interventions and programs in designated ambulatory practices
- Monitors and evaluates efficacy of individual practice-redesign interventions and programs and adjust accordingly
- Develops, implements, tracks, trends and synthesizes overall data analysis to inform practice improvement strategy
- Educates practices regarding ICP’s clinical practice improvement initiatives as well as ICP’s engagement and quality incentive program
- In conjunction with ICP’s leadership, identifies practice and provider needs and produces summary reports that help guide provider engagement and practice decisions
- Effectively collaborates with peers, especially the ICP Provider Relations Specialists to provide coordinated care and customer service to ICP practices. Ensures tight alignment to meet the needs of the practices
- Incorporating front-line experiences, provides recommendations to Manager and/or Director for improving reporting tactics and processes in order to more effectively support ICP’s population health and quality initiatives

Reports To: Manager of Practice Transformation, Integrated Care Partners
Nature of Supervision:
- Internal: Will engage with the ICP clinical team, health economics team, performance management board subcommittee, HHC inpatient case management, HHC clinical team leaders
- External: Interacts with ICP members
Qualifications
Education: Must have a Bachelor’s
degree or equivalent relevant experience in health related field.
Experience: At least 3-5 years’ health care related
experience.
Knowledge, Skills and Ability
Requirements:
- Strong working knowledge of
medical practice operations
- Ability to influence practice
changes to discrete and varied audience i.e. Administrators, practice
managers, clinicians as needed
- Ability to innovate to achieve
the organization’s goals
- Ability to understand and
convey complex information to many difference kinds of people is an
absolute requirement with demonstrated ability to work as a member of a
team
- Experience with electronic
medical records (Athena, EClinical Works, AllScripts, Epic etc) preferred
- Experience with hospital
systems (Cerner, Sunrise, Epic and Premier) preferred
- Excellent communication,
analytical, presentation and interpersonal skills
- Competence in quality
improvement and data interpretation
- Familiarity with case
management and care coordination processes; experienced and skilled in
working with interdisciplinary teams
- Comfortable using and/or
learning to use databases, reporting software, electronic medical record
- Exhibits leadership in
demonstrating to staff how to embody the patient-centered goals of the
organization, and in embracing change, where appropriate
- Proficient in Excel,
PowerPoint, Access. Experience with Tableau highly preferred
