Job Summary

 

Provides prescribed medical treatment and personal care services to patients of Charlotte Hungerford Hospital. Implements, follows and monitors patient’s treatment plans. Responsible for providing clinical support and case management services to support physicians and APRN’s in Outpatient Behavioral Health settings. Responsibilities include approved patient care functions; clerical, environmental, and organizational tasks, including data collection and data entry. The Licensed Practical Nurse provides information to patients so they may fully utilize and benefit from services available within the behavioral health service line or community agencies.

 

Job Responsibilities

 

  1. Daily operational support for the medication prescribing and refill processes for physician and APRN providers in ambulatory behavioral health.  The LPN will provide oversight to Medical Assistants who also work under the direction of the Medical Director.
  2. Under the direction of a physician, provides direct patient care to meet individual treatment plan goals by obtaining, documenting and reporting data related to the established plan of care. Documents and reports to the physician observations of patients vital signs, symptom, behaviors and response to medication.
  3. Fulfills clerical responsibilities as assigned that may include sending/receiving patient medical records; obtaining lab/X-ray reports, hospital notes, referral information; completing forms/requisitions as needed; scheduling appointments; verifying insurance coverage; securing insurance approval for medications; and  managing and updating the electronic medical record to ensure that information is complete and filed appropriately.
  4. Assists with patient and family education as per treatment plan.
  5. Take telephone requests for medications; collect medical information related to refill request; verify medications requests; place orders for providers related to medications; e-prescribe/call in medication refills for non-controlled substances with a providers order; and inform patients of normal test results.

Qualifications

Qualifications

  • High School diploma or GED

  • Completion of an LPN program

  • One or more years of physician's office experience

  • Knowledge of health care field and medical office protocols/procedures. 

  • Knowledge of specific assisting tasks related to particular medical practice and behavioral health patient needs

  • Use of standard medical exam/office equipment

  • Skill in interpersonal and supportive communication skills

  • Skill in understanding patient education needs by effectively sharing information with patients and families

  • Ability to plan, prioritize, and complete designated tasks
