St. Vincent’s Medical Center in Bridgeport has more than 3,200  employees. It includes a 473-bed community teaching hospital, a 76-bed inpatient psychiatric facility in Westport, a large multispecialty provider group, and special needs services for adults and children. Job Summary:

  • Physical therapist position in new Outpatient Spine Wellness center; this therapist will be part of a multidisciplinary team of medical, rehabilitation, and wellness professionals dedicated to improving the health of all patients. This therapist will collaborate with professionals throughout the facility who provide rehabilitation, pain management, physical medicine, integrative medicine, fitness, nutrition, and behavioral health services to improve surgical and non-surgical outcomes. These services will be provided within the spine wellness center with the goal of promoting health of mind and body.  This therapist will interact with rehabilitation team members throughout the hospital system to ensure that best practices are followed in the provision of care.  Data will be collected and analyzed to improve patient outcomes, and the therapist will partner with program managers to implement, enhance, and optimize clinical programs. The physical therapist will:  independently evaluate and determine treatment strategies, deliver and/or delegate services that identify, prevent, remediate, and rehabilitate acute or prolonged physical dysfunction or pain.  This role encompasses evaluation skills and techniques, analysis of patients, and therapeutic application of physical and chemical agents, exercise and other procedures to maximize the patient's functional independence.  Evaluation involves strength, motor control, sensation, joint mobility, soft tissue integrity, pain, balance, coordination, posture, cardiopulmonary function, endurance, psychosocial contributors, gait, and functional abilities in activities of daily living.  This professional will collaborate extensively with team members, referral sources, and colleagues throughout the system of care to enhance the provision of services and patient outcomes. 

Responsibilities:

  • Performs evaluations for strength, motor control, joint mobility, soft tissue integrity, pain, balance, coordination, posture, cardiopulmonary function, endurance, gait and functional abilities in activities of daily living through physician referrals.  Recognizes physiological, psychological, pathological, and age related changes in the patient; consults with other Health Care Team members, and establishes measurable goals with the patient/family based on appropriate patient care protocols and critical paths in order to meet patient needs.
  • Plans, implements, delivers and delegates patient care by utilizing appropriate interventions, such as heat, cold, light, water, sound, electricity, mobilization, manual therapy, and therapeutic exercise based on assessments, revising treatment when needed, instructing patient/family through verbal, written and audiovisual materials and participating in discharge planning including recommendations to community resources to meet patient needs throughout the continuum of care.
  • Coordinates time management of self and support personnel by organizing schedules to meet patient care needs, carrying a patient load that reflects standards of the service area, and collaborating with team members for assistance and coverage when needed to meet patient outcomes and department goals and standards.  Supervises responsibilities that have been delegated to supportive personnel.  May cover patient care needs in other facilities if necessary, and will serve as mentor for new employees and student physical therapists. 
  • Documents all patient and professional interactions such as evaluations, assessments, progress towards goals, equipment purchase orders, statistical reports, home programs and discharge summaries to maintain continuity of patient care for department standards, JCAHO and outside regulatory and insurance agencies.
  • Communicates understanding both verbally and nonverbally to patients and families, health care team members, supervisors, and students in an appropriate manner to enhance positive relationships both internally and externally.  Demonstrates professionalism in all interactions to promote the needs of the patient and the institution they represents.



Qualifications

Licensure / Certification / Registration:

• Required Credential(s):
• Physical, Occupational or Speech Therapist License in the state of CT

Education:
•  Bachelor’s Degree from an accredited therapy program or Master’s or Doctorate degree

Work Experience:
•Experience in outpatient setting preferred, will consider qualified new graduates or Experience in outpatient setting, with focus upon spine rehabilitation and manual therapy



Knowledge, Skills and Ability Requirements:

  • Computer proficiency in Microsoft Word, Excel, PowerPoint as well as electronic health records and other software applications
  • Ability to establish and maintain relationships with referral sources
  • Effectively manages patient case load in a fast-paced, collaborative environment
  • Ability to solve problems and work independently with minimal direction
  • Strong leadership, mentoring, and motivational skills
  • Strong sense of professionalism and proficient interpersonal, clinical, and communication skills
  • Commitment to quality, evidenced based care, and client experience
