Shift Details:

The consultant works with all levels of the organization to deploy the Hartford Healthcare at Home (HHCAH) quality assurance and quality improvement strategy.  Working with Management and H3W facilitators, the consultant will focus on continuous improvement and the integration of identified improvement initiatives throughout the organization.  The consultant will act as a change agent, partnering with management and staff to identify best practices and achieve higher level results by utilizing and implementing improvement processes in assigned work groups and organization-wide.

 

In general, most of the time will be spent in the following activities:

 

Support the development and implementation of improvement and change processes as defined by the Director of Quality and Education, and Senior Leadership.

 

Uses a data-driven, pro-active approach to improving the quality of patient experiences, care and services

 

Considers HHCAH compliance with Federal Conditions of Participation (CoP), state regulation, plans of correction, and best practices when coaching and mentoring management and staff. Assists in selection of drivers and countermeasures to improve quality of care, safety and compliance.

 

 

 

Promotes a high level of participation and contribution of individuals or groups, enabling individuals/groups to assume responsibility for outcomes.

 

Understands the inter-relationship among broad organizational goals and strategies, such as High Reliability, H3W and organizational values, while maintaining focus on achieving strategic objectives. 

 

Works effectively as a team member within the department and other units to provide support through pertinent knowledge, communication, cooperation and collaboration.

 

Monitors assigned performance improvement efforts to ensure high quality results in an efficient manner.

 

Facilitates consistency, collaboration and communication across the organization.

 

Provides training, on an as needed basis, in the use of quality improvement tools like flow charting, Pareto analysis, run charts, histograms, etc.

 

Directs local clinical record review teams for accurate data abstraction.

 

Collates QAPI data and forwards to the Director of Quality for data analysis.

 

Uses visual displays of performance data to assist and inform workgroup leaders.

 

Assist managers and staff to anticipate and deal with performance improvement barriers as they arise.  

 

Actively participates in QAPI committees

 

Performs data abstraction to idesntify opportunities for improvement in Star ratings.

Qualifications

Minimum Qualifications:  

 

Bachelor’s Degree required; Master’s Degree in nursing or therapy preferred.  OASIS certification required. Five years or more of progressive work experience in health care required or similar progressive work experience in Quality/Process Improvement, Organization Development and Training or a related discipline, case management in chronic care models.  Obtain lean training within one year of employment. Experience in facilitation and presentation required.  Microsoft Office proficiency required, including Outlook, Excel, Word, and PowerPoint.
