DescriptionJob Schedule: Part Time
Provides overall delivery of TM services and maintenance of clinical tele-monitoring equipment for patient use
Responsibilities include the following activities:
** Home visits to delivery, set-up and instruct patient and family in using the TM equipment
** Provides consultation to staff identifying and planning interventions designed to solve TM problems involving patients care
** Assists in providing educational programs for TM program aimed to improve clinical knowledge of direct service staff
** Participates in an interdisciplinary approach to client care for patient using of TM equipment
** Participates in educational programs to train and coach new agency staff.
** Maintains and logs equipment per guidelines for quality control and maintenance of equipment
** Coordinates home visits and discharges of patients from the TM program
** Will actively participate in the performance improvement process known as H3W.
** Performs other duties as assigned

Education: High School Diploma, Associates degree preferred
Experience: One- two years’ experience in home care preferred
