Flexible PER DIEM Internal, Family or Geriatric Medicine Opportunity in Connecticut

Hartford Healthcare Medical Group (HHCMG) is seeking a board certified/board eligible internal medicine, family medicine or geriatrics trained physician to join our growing Geriatrics program. Built on an institute model, our program currently provides Geriatric care, consultation and expertise to geriatric patients in various ambulatory and facility settings.

We are seeking an experienced physician to cover Skilled Nursing Facility rounding to complement our full-time provider staff and to cover when our full-time provider staff is on vacation or short-handed.

Position details:

• Per diem (as needed) coverage in the Skilled Nursing and Assisted Living settings

• No call coverage

• Ideal for physicians seeking a flexible schedule

Location(s):

• Greater Southington, Farmington, Bloomfield,  and/or Torrington areas

Requirements:

• Experience and a passion for providing patient-centered care to a geriatric patient population focused on quality, prevention and education

• A willingness to work closely and collaboratively within an interdisciplinary team

• An unrestricted State of Connecticut Medical License (or eligible)

• Board Certified/Board Eligible Internal, Family or Geriatric T Medicine Physician
