Position Details:
Hartford HealthCare is seeking an experienced, Primary Care, Nurse Practitioner for Primary Care coverage for several practices in the greater West Hartford area. The work hours are weekdays only and the schedule is flexible and based on mutual agreement.
Role: Open to Nurse Practitioners (FNP, ANP, AGNP)
Location: Greater West Hartford
Setting: 100% Outpatient Primary Care – Adult Medicine
Schedule: Per Diem, weekdays only, schedule based on mutual agreement, no minimum hours
Call schedule: No Call required

Nurse Practitioners Role Scope:
Under established guidelines and in collaboration (if required) with the practice physician(s), Nurse Practitioners perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.
In consultation with the physician(s) and care team members, Nurse Practitioners develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.
Nurse Practitioners provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.
Qualifications
Requirements:
- Licensure: Unrestricted State of Connecticut Medical License (or eligible)
- Training: APRN (Family, Adult/Geri Nurse Practitioner)
- Experience: At least two years of post-graduate experience as a Primary Care Nurse Practitioner (APRN)
