Job Summary

Delivers efficient, high quality and safe nursing care to all clients.. Monitors, observes, intervenes with the medical/nursing management of clients’ care.

Job Responsibilities

  1. Gathers data to assist the registered nurse with the assessment process.  Schedules medical appointments and coordinates weekly psychiatric consults with ITP clinicians. Collaborates with psychiatrist/APRN regarding weekly consults and medications and documents clients’ medical conditions.
  2. Promotes minimal disruption with established, effective, chronic medical and psychiatric intervention under the direction of the Registered Nurse (RN).
  3. Communicates to clients the intended effects of medications being administered. Communicates to the RN the actual effects experienced by the client to obtain timely modification of medication orders and RN re-assessment by the RN when desired effects are not achieved.
  4. Administers medications to clients according to the MD’s/APRN’s standing orders. Monitors and reports on any negative outcomes, as per policy. Serves as a liaison between the Pharmacy and the program.
  5. Prepares appropriate medical documentation to meet regulatory requirements of licensing and third party agencies within his/her scope of practice and under the supervision of a RN.
  6. Ensures clients’ needs are met and notifies the charge nurse or the Director of Nursing if a client feels that his/her needs have not been met.


  • Completion of a licensed LPN program.
  • Valid CT LPN license.
  • Minimum of one years’ experience in Medical/Surgical, Critical Care, Psychiatric or Substance Abuse Treatment preferred.
  • CPR certified within six months of employment.
